The heart transplant RN Coordinator plays a pivotal role in the multidisciplinary management of patients undergoing heart transplantation,
This position is responsible for the comprehensive coordination of care
Reports To: Transplant Program Manager
Qualifications:
Required:
- BSN Degree
- Graduate of an NLN accredited school of nursing required
- Minimum 3 years of clinical experience, preferably in cardiology, critical care, or transplant nursing
- Strong clinical assessment skills and ability to work independently,
- Strong cardiac background
- Heart Transplant experience
- Ability to navigate complex emotional and ethical situations with empathy and professionalism
Preferred :
- MSN Degree
- Experience with transplant databases and EPIC EMR system
- Knowledge of UNOS and CMS transplant regulations
Certifications and Licenses Required:
- Active NJ RN license or compact RN license with NJ endorsement
- BLS from the American Heart Association
- ACLS from the American Heart Association
Scheduling Requirements :
- Day Shift, 8:30a - 4:30p (variable and not guaranteed)
- Full-Time, 37.5 hours
- Monday – Friday, position requires on call /and weekend and holiday hours
Essential Functions:
The Heart Transplant RN Coordinator plays a pivotal role in the management of patients undergoing heart transplantation,
This position is responsible for the comprehensive coordination of care across the transplant continuum: including preoperative evaluation both in patient and outpatient, inpatient hospitalization follow up and education, and management of listing criteria,
The RN Coordinator ensures that patients receive timely, evidence-based, and compassionate care while serving as a liaison between patients, families, and the transplant team
Key Responsibilities includes:
Preoperative Evaluation
- Conduct thorough assessments of potential heart transplant candidates, including medical history, psychosocial factors, and diagnostic testing
- Collaborate with physicians, social workers, dietitians, and financial coordinators to determine transplant eligibility
- Educate patients and families about the transplant process, risks, benefits, and expectations
- Coordinate pre-transplant testing, consultations, and multidisciplinary review meetings
Inpatient Management
- Serve as the primary nursing contact during hospitalization for transplant surgery
- Monitor patient progress, coordinate care plans, and facilitate communication among care teams
- Support patients and families through the surgical experience, including pre-op preparation and post-op recovery
- Ensure compliance with transplant protocols and regulatory standards
Follow up
- Provide ongoing education and support to patients and caregivers regarding lifestyle modifications and warning signs of rejection or infection
- Coordinate outpatient visits, diagnostic testing, and communication with referring providers
- Maintain accurate documentation and participate in quality improvement initiatives
Other Duties:
Provide clinical expertise to all hospital departments care of the patient and patient management. Provide day to day patient management to ensure quality care and mentor inpatient nurses in the care of this patient population. Work with nursing education to provide annual refresher education and competence. Manage patients in the outpatient setting. Work with health care providers in patients’ local community to recognize the device and how to provide basic support. Collaborate with research staff to ensure all protocols are followed. Work with the Administrative Director to ensure patient care and program policies meet or exceed regulatory expectations at all times
