We are seeking a compassionate and detail-oriented Registered Dietitian (RD/RDN) to provide clinical nutrition oversight across a portfolio of 10 skilled nursing facilities.
This regional role partners with facility leadership and interdisciplinary teams to ensure high-quality resident care, regulatory compliance, and consistent nutrition standards across all centers.
Responsibilities
• Perform nutrition assessments for admissions and high-risk residents
• Develop and monitor individualized nutrition care plans
• Track weight trends, labs, intake, and hydration
• Participate in care plan meetings (on-site and virtual)
• Support CMS and state regulatory compliance
• Conduct routine facility visits and assist with survey readiness
Requirements
• Registered Dietitian (RD/RDN)
• Active state license (if applicable)
• Long-term care experience preferred
• Ability to travel between facilities
Full-time | Travel required across 10 buildings
