• 2 years of acute care experience strongly preferred
  Case Management, Managed Care.
• In Patient Case Management is required
  BSN required
  RN license required
  CCMC preferred

Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process Performs utilization management and quality screenings In- Patient Case Management is required
