Overview
We are seeking a compassionate and skilled Registered Nurse (Hospice) to join a well-established home health and hospice team. This role allows you to make a meaningful difference by focusing on one patient at a time in the comfort of their home. Responsibilities

• Deliver professional nursing care to patients in their homes through assessment, planning, implementation, and evaluation.
• Ensure compliance with physician orders and document progress toward goals.
• Provide holistic, safe, and effective care using evidence-based practices.
• Collaborate with interdisciplinary teams, physicians, and family members to coordinate services.
• Complete nursing assessments, medication reconciliations, and updates to the plan of care.
• Educate patients and families on treatment plans and self-care strategies.
• Supervise home health aides, conduct regulatory supervisory visits, and support their development.
• Maintain accurate and timely documentation in accordance with regulatory standards.
• Demonstrate flexibility in job responsibilities and adhere to organizational standards of practice.

Qualifications:
Required:

• Associate's Degree in Nursing or Nursing Diploma
• Current New Jersey RN License
• AHA Basic Life Support (BLS) Certification
• Minimum 6 months RN experience OR 1 year LPN experience (home health or hospice preferred)
• Basic computer proficiency

Preferred:

• Bachelor of Science in Nursing (BSN)
• IV Certification
• Prior home care or hospice experience
