Shifts: 8a-4p; 5 days per week; On call 1 weekend per month

This role involves providing comprehensive hospice care, focusing on patient and family needs. The successful candidate will work in various counties, ensuring a supportive environment for patients receiving end-of-life care.

Key Responsibilities of the RN - Hospice - Travel

• Performs patient assessments for hospice care suitability, develops care plans
• Ensures appropriate utilization of services and maintains visit productivity
• Completes discharge assessments and revises care plans as necessary
• Uses age-appropriate teaching methods and documents educational tools
• Assumes responsibility for case management and prioritizes care needs
• Maintains patient safety and infection control standards
• Attends and participates in interdisciplinary team meetings
• Contributes to the agency's growth by training new employees

Requirements of the RN - Hospice - Travel

• Current New Jersey RN License
• 2-3 years of RN experience in a home hospice environment
• Valid New Jersey driver's license
• BLS certification
• Experience using EMR systems
• Excellent critical thinking and communication skills
