Affirmed Home Care is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to deliver exceptional in-home healthcare services throughout Northern and Central New Jersey .

Key Responsibilities

• Provide skilled nursing care under the supervision of an RN and in accordance with care plans

• Administer medications, treatments, and wound care as prescribed

• Monitor and document patients’ conditions, reporting changes promptly to the RN or supervisor

• Assist with IV therapy, catheter care, and other skilled nursing tasks as permitted by scope

• Educate clients and families on medication adherence and home safety practices

• Maintain accurate, timely, and compliant documentation in EHR systems

• Promote a safe, respectful, and supportive environment for clients and families

Qualifications

• Active LPN License in the State of New Jersey (required)

• 1–2 years of clinical experience preferred (home care, long-term care, or related field)

• Excellent communication, assessment, and time-management skills

• Compassionate, dependable, and able to work independently in a home care setting

• CPR/BLS certification (required)

• Valid driver’s license and reliable transportation

• Experience with Electronic Health Records (EHR) preferred
