Job Description
Responsible for recruitment, information and referral to resources for ex-offenders. Maintaining contact with clients in the community in an effort to offer support and guidance and linking clients with needed services to promote employment, treatment, and housing.
Scope of Duties:
- Recruit clients to participate in program including visiting the county jail to interview prospective clients.
- Complete intake interviews with clients and secure program participation agreements.
- Assist clients apply for social entitlements such as GA, TANF, etc. when appropriate.
- Link homeless clients to housing.
- Monitor and document client progress.
- Assess client needs and work with client to develop goals.
- Update service plan with client input.
- Provide motivational support to clients in program and maintain support throughout participation.
- Schedule and facilitate meetings with clients as required.
- Identify and/or develop resources and link clients to needed services.
- Facilitate scheduled workshops/programs/groups addressing client's needs.
- Develop appropriate discharge plan on all assigned clients.
- Participate in required internal/external meetings.
- Complete all required reports, data collection and other documentation.
- Interact with all clients, staff, and outside contacts with respect and courtesy.
- Maintain client and staff confidentiality in compliance with administrative policies and procedures.
- Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
- Other duties as assigned.
ADA Essential Functions:
- Occasionally required to smell, stoop.
- Regularly required to use hands to handle or feel.
- Regularly required to reach.
- Regularly required to stand, walk, sit, talk and hear.
- Regularly required to lift and move up to 10 lbs.
- Ability to see clearly at 20 inches or less.
- Ability to observe an area that can be seen up and down or to the left and right while remain fixed on a given point.
