Hadiatou Diallo’s Primary and Mental Health Care LLC is expanding and actively seeking compassionate and dedicated Nurse Practitioners to join our growing team. We are offering both full-time and part-time opportunities within our skilled nursing facility practices across New Jersey.

✅ Qualifications:

• Active Nurse Practitioner license in the state of New Jersey

• Minimum 1 year of experience in a nursing home or long-term care setting preferred

• New graduates are welcome to apply – we provide strong mentorship and ongoing clinical support

Key Responsibilities:

• Provide high-quality primary care services in collaboration with supervising physicians

• Perform patient assessments, diagnoses, and manage treatment plans

• Communicate and coordinate effectively with interdisciplinary care teams to ensure quality patient outcomes

• Complete timely and accurate documentation to support compliance, audits, and billing processes
