Regional Registered Dietitian (RD) - Skilled Nursing
Position Overview
We are seeking an experienced Regional Registered Dietitian (RD) to provide clinical nutrition oversight and support across multiple skilled nursing and rehabilitation centers. This role partners closely with nursing leadership, dietary teams, therapy, and medical providers to ensure resident-centered, evidence-based nutrition care that supports clinical outcomes, regulatory compliance, and quality measures.
The ideal candidate is clinically strong, organized, approachable, and comfortable working independently across multiple sites—balancing direct resident care, regulatory readiness, and team education.
Key Responsibilities
Clinical Nutrition & Resident Care
- Complete and oversee comprehensive nutrition assessments, care plans, and progress notes in accordance with CMS, state, and professional standards
- Identify, monitor, and manage nutrition-related risks including weight loss, dehydration, malnutrition, dysphagia, wounds, renal disease, diabetes, and therapeutic diets
- Collaborate with interdisciplinary teams (nursing, therapy, physicians, MDS, and dietary staff) to support optimal resident outcomes
- Provide recommendations for supplements, modified textures, and specialized diets aligned with IDDSI standards
Regulatory & Quality Oversight
- Ensure compliance with CMS regulations, state survey requirements, and facility policies related to nutrition services
- Support survey preparedness and participate in mock surveys, audits, and corrective action plans
- Review quality indicators related to nutrition, weight trends, wounds, and hydration
- Assist centers with documentation accuracy to support Quality Measures and Five-Star performance
Leadership & Education
- Provide ongoing education and mentorship to dietary managers, cooks, and clinical staff
- Support onboarding and competency development for dietary teams
- Serve as a regional clinical resource for nutrition-related questions and best practices
- Promote a culture of resident choice, dignity, and person-centered care
Operational Support
- Travel to assigned centers on a scheduled basis with flexibility for priority needs
- Utilize electronic health records effectively and support documentation consistency
- Communicate clearly with regional and corporate clinical leadership on trends, risks, and opportunities
Qualifications
Required
- Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration
- Active state licensure (or ability to obtain) in assigned region
- Minimum of 2–3 years of experience in skilled nursing, long-term care, or post-acute care
- Strong knowledge of CMS regulations, MDS processes, and survey expectations
- Excellent communication, organization, and time-management skills
- Ability to travel regionally and work independently
Preferred
- Experience in a regional or multi-site role
- Familiarity with IDDSI diet standards
- Experience supporting quality initiatives, Five-Star measures, and survey remediation
- Comfort with change management and staff education
