The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

The Heart Failure RN, will collaborate with the clinical care team in the ambulatory setting managing patients with advanced heart failure,

Bachelors degree RN required,

1-3 years of related experience within the healthcare field, required,

Ability and interest to learn clinical information and criteria,

Has experience with heart failure population

Emphasis on computer literacy and communication skills, such as Microsoft Excel and Word,

Participates in the development and implementation of health promotion programs for patients and their families

Designs health maintenance and disease prevention interventions for patients

Provides anticipatory guidance and counseling of patients/families and assists them to cope with competing concerns of multi-generational family members

Evaluates patient s and caregiver s support systems

Facilitates patient s transition between health care settings, such as hospital to home or to a skilled nursing facility

Manages commonly occurring acute and chronic physical and mental health issues

Patient Management: Heart Failure

Identifies that patient assigned is appropriate for Heart Failure team management

Appropriately identifies diastolic and systolic heart failure and their management

Appropriately identifies different cardiomyopathies

Correctly interprets results of diagnostic studies

Accurately interprets hemodynamics from right heart catheterization or indwelling pulmonary artery catheter

Ensure accurate medication reconciliation upon admission

Provides individualized HF education to the patient and his/her caregivers

Activate Heart Failure pathway

Provides appropriate referrals

Validates that patient is receiving guideline directed medical therapy

Appropriate collaboration with attending physician, consulting cardiologist, or other specialties as needed,

Ensure accurate medication reconciliation upon discharge
