Location: Hudson, NY
Type: Contract
Shift & Schedule: Day Shift | Monday – Friday | 8:00 AM – 4:00 PM
Pay Rate: $30 to $40.00/hr(Based on Experience)
Position Overview
We are seeking an experienced Registered Dietitian to provide comprehensive nutrition care for residents in a long-term care setting by developing individualized nutrition plans, promoting resident well-being, and collaborating with the interdisciplinary care team.
Responsibilities
Conduct comprehensive nutrition assessments, including Nutrition Focused Physical Exams (NFPE), develop, implement, monitor, and evaluate individualized nutrition care plans using the Nutrition Care Process.
Provide nutrition counseling and education to residents and families, perform meal observations, address cultural, religious, and age-specific nutrition needs, and coordinate referrals for outpatient nutrition services when appropriate.
Collaborate with nursing, dining services, and the interdisciplinary care team to deliver person-centered nutrition care, participate in care planning, risk, weight, and wound meetings, and ensure compliance with Aramark SeniorLIFE+, regulatory, and client standards.
Maintain accurate documentation in medical records, care planning, and menu management software, monitor clinical productivity, protect resident health information, and conduct sanitation, dining, and tray line audits to support quality improvement.
Required Qualifications
Bachelor's degree in Nutrition, Dietetics, or a related field from an accredited institution.
Registered Dietitian credential through the Commission on Dietetic Registration (CDR).
Current state licensure and continuing education requirements as required by state law.
Experience with nutrition assessments, Nutrition Care Process (NCP), NFPE, nutrition counseling, care planning, documentation, interdisciplinary collaboration, regulatory compliance, and quality improvement.
Preferred Qualifications
Previous experience working with older adults in a skilled nursing facility or long-term care community.
