The Homemaker-Home Health Aide (HHA) provides assistance and performs supportive tasks to maintain the well-being of an individual within his/her own environment; assists clients with activities of daily living to include personal care, nutrition, ambulation ,homemaking, respite, companion and socialization activity; adheres to safety precautions, infection control techniques, and HIPAA regulations; and performs all activities under direction and supervision of a Registered Nurse and in accordance with the Code of Conduct . DUTIES AND RESPONSIBILITIES
The Homemaker-Home Health Aide 1. Provides care in accordance with HHA scope of service for providing personal care to include
a. Bathing and hygiene
b. Assisting with grooming and dressing
c. Toileting
2. Provides assistance with activities of daily living (ADL)
3. Assists with ambulation and transfers
4. Prepares meals following dietary instructions and assists with feeding as appropriate
5. Assists client with shopping needs and errands
6. Performs simple health tasks in accordance with a plan of care/service such as: reminding patients to self-administer prescribed medications, range of motion exercises, use of special equipment, and vital signs as instructed by RN
7. Performs household and light housekeeping tasks such as tidying kitchen, dishwashing, linen changes, laundry, etc.
8. Maintains a clean and safe environment within the client's immediate areas of activity
9. Implements basic infection control procedures in accordance with agency policies
10. Receives assignments and reports on tasks completed and need for changes
11. Reports and documents changes in the patient's condition, any unusual occurrences, and accidents to the Nursing Supervisor
12. Performs other supportive tasks in accordance with a care plan outlined by the Nursing Supervisor
13. Completes timely, accurate, and complete documentation of care/service performed weekly in accordance with organizational policy
14. Demonstrates understanding of the Client Bill of Rights and respect for the clients privacy and property
15. Complies with HIPAA regulations assuring confidentiality in all aspects of client, employee, and agency information.
16. Communicates clearly and effectively with clients, coworkers, and supervisory staff
17. Submits annual health screening, credentialing, and education update documentation required for HR record prior to expiration dates
18. Completes required in-service training and competency evaluations
19. Participates in Performance Improvement activities
