Residential Case Manager

$2000 Sign On Bonus (paid after 12 months per policy)

We are seeking a qualified applicant to work in our Residential program as a Case Manager.

The Residential Case Manager provides supervision and support to clients living in a group home or apartment/community setting. They encourage residential client's wellness and recovery through group activities, education in daily living skills, supportive counseling, and medication management. They participate in the development of Treatment Plans for each client on caseload, as well as providing oversight, documentation, and coordination of client's receipt of medical care benefits and all Psychiatric rehabilitation services.

Flexible Hours 35 - 40 per week, inclusive of days, evenings and weekends.

Qualifications: BA degree from an accredited college or university with a major in a mental health, health, or other appropriate human services related discipline; or a licensed registered nurse; or a combination of one or more years of college plus related work or life experience together equaling four years (The individual is required to have at least one year of related work or life experience, if the four year college education does not meet the requirements above); or a license as a practical nurse plus two years of related work or life experience; or a high school diploma or the equivalent, plus four years of related work or life experience. Additionally one year experience in a residential mental health setting is required. CPR/First Aid trained, to be obtained prior to providing services alone and within 30 days of hire. We will provide training if none currently exists. A valid drivers license, meeting the recommendations of the Agency Auto Insurer is preferred.
