Shift: Night Shift (Mixed 8 & 12 Hours)
Schedule: 4 Shifts/Week (36 Hours Total) – Every Other Weekend Required
Typical Hours:
- Night: 11:00 PM – 07:00 AM (8 Hours)
- Evening: 03:00 PM – 11:00 PM (8 Hours)
- Extended Night: 07:00 PM – 07:00 AM (12 Hours)
(Schedule includes 3x8-hour shifts + 1x12-hour shift per week based on unit needs)

Pay: $46/hour (W2) + $1244 Weekly Stipend
Weekly Gross: $2900

Job Summary
The Long-Term Care Registered Nurse (RN LTC) – House Supervisor is responsible for overseeing clinical operations across a 180-bed skilled nursing facility during off-shifts, including evenings, nights, weekends, and holidays. This leadership role requires strong clinical expertise, decision-making skills, and the ability to coordinate patient care and staffing across multiple units.
The RN Supervisor serves as a central resource for nursing staff, providing guidance, conflict resolution, and clinical support while ensuring high-quality resident care. The facility includes five specialized units caring for residents with a wide range of conditions, including dementia, Alzheimer’s disease, post-surgical rehabilitation, cardiac conditions, diabetes, and stroke recovery.
The supervisor ensures smooth patient flow, manages emergencies, supports staff performance, and maintains compliance with regulatory and safety standards.
Roles and Responsibilities
- Supervise and coordinate nursing care across all units within the LTC facility
- Serve as the primary clinical resource for staff during off-shifts
- Provide leadership, mentoring, and coaching to nursing personnel
- Facilitate patient admissions, transfers, and discharges
- Monitor resident conditions and intervene in acute or emergency situations
- Lead and manage code situations and emergency response procedures
- Ensure proper staffing levels and coordinate centralized staffing adjustments
- Support de-escalation and management of aggressive or assaultive behaviors
- Oversee infection control practices, isolation protocols, and workplace safety
- Ensure compliance with HIPAA, OSHA, DPH, and Joint Commission standards
- Manage incident reporting, risk management, and sentinel event documentation
- Assist with end-of-life care processes, including documentation and family communication
- Monitor and maintain use of specialized equipment such as wound vacs, PICC lines, tube feeds, and catheters
- Perform or assist with IV therapy and vascular access management as needed
- Collaborate with interdisciplinary teams to optimize resident care outcomes
- Ensure accurate documentation in EPIC EMR
- Maintain accountability for regulatory compliance and quality standards across all units
Unit Overview
- Moore Place: Neurobehavioral unit (dementia/Alzheimer’s with behavioral challenges)
- Bilz Unit: Rehabilitation (post-orthopedic and cardiac procedures)
- Vantassel Unit: Secure dementia unit (elopement risk residents)
- Schulz Unit: Long-term care residents
- Wolf Unit: Long-term care residents
Average Daily Census: 180 Residents
Qualifications
- Active RN License (New York State) required
- BLS Certification required
- Minimum 2 years of LTC or skilled nursing experience required
- Previous Charge Nurse or Supervisory experience required
- Must have recent clinical experience within the past 6 months
- EPIC EMR experience required
- First-time travelers accepted
- Must reside more than 75 miles from facility (local candidates not accepted)
- Strong leadership, critical thinking, and communication skills
- Ability to manage multiple units and prioritize in a fast-paced environment
