Location: Atlantic & Cape May Counties, New Jersey​

Schedule: Mon - Fri 8:30-5:00 pm

Role Overview

As a key member of our Interdisciplinary Team (IDT), you will play a pivotal role in delivering holistic care that meets the physical, emotional, and spiritual needs of patients and their loved ones. Your role involves initiating appropriate interventions upon admission and ensuring a continuous, responsive plan of care throughout the patient's journey.​

Primary Responsibilities

• Deliver skilled nursing care as outlined in individualized care plans, ensuring comfort and quality of life for terminally ill patients. ​

• Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust patient care plans as needed.

• Educate and support patients' families and primary caregivers, providing guidance on care techniques and offering emotional support during this challenging time. ​

• Serve as a liaison between patients, families, and other healthcare providers, ensuring clear communication and advocating for patients' wishes and needs. ​

Qualifications

• Valid New Jersey or Multi-State Registered Nurse (RN) license.​

• Minimum of two (2) years of nursing experience in acute-care settings such as medical, surgical, or oncology units. ​

• Strong clinical practice with a focus on customer service, excellent communication abilities, and the capacity to work independently in a fast-paced environment.​

• Ability to work autonomously while contributing to a collaborative, interdisciplinary care model
• Reliable transportation and willingness to travel throughout Atlantic County
