Responsibilities:
- Completes assessment and evaluation of patient's impairments and deficits.
- Working hand in hand with our current therapist.
- Creates a treatment plan and goals to address the impairments and deficits with consideration of evaluation and assessment, medical status, and prior level of function. The treatment plan and goals are monitored and adjusted based on the patient's needs and progress.
- Advises patient on proper form and makes sure the patient will not injure themselves during exercise.
- Instruct, encourage, and assist patients in performing a therapeutic activity, therapeutic exercise, and activities of daily living, specific to each patient's needs and goals. Adjusts selected therapeutic activity, therapeutic exercise, and activities of daily living based on the patient's response to the activity.
- Evaluate the effects of treatments by observing, noting, and evaluating the patient's progress and recommending adjustments and modifications in consultation with other providers.
- Prepares written documentation such as notes, reassessments, and discharge summaries.
- Documentation includes monitoring the patient’s status and progress toward goal achievements. Re-evaluations are completed in a timely manner.
- Goals and treatment plans are updated as per department policy. Consistently, accurately, and independently prioritizes and completes routine assignments within guidelines and schedules.
- Board-certified and licensed in Physical Therapy by the New Jersey Board of Medical Examiners.
- Possess excellent interpersonal and communication skills, with the ability to empathize with and relate to patients.
- $60-$65 hourly (depending on experience)
