Responsibilities
Elwyn seeks a Program Specialist to ensure the delivery of high-quality, person-centered services to individuals with intellectual and developmental disabilities. This role oversees compliance with state regulations, agency policies, and individualized service plans (ISPs) within the Community Living Program (CLP) which are provided to meet the needs of the individuals receiving services.
The Program Specialist contributes to the development of the ISPs and is responsible for coordinating, implementing, and monitoring ISPs for an assigned caseload. This role ensures all ISP outcomes are achieved and that individuals’ health and safety needs are met. The Program Specialist ensures continuous development while monitoring programming and services for individuals supported. The Program Specialist ensures that services are implemented in accordance with each individual’s ISP while providing ongoing training, coaching and support to the direct support professionals, to maintain compliance with regulatory requirements.The hourly range for the role is between $27-$28/hr, negotiable dependent on background, experience, and education.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Coordinates services and supports for individuals by serving as the primary liaison between families, legal guardians, community providers, representative payees, and other stakeholders.
- Assesses the individuals' care needs through documentation review to ensure that the team addresses them effectively.
- Monitors on-going implementation of ISPs by reviewing daily documentation, staff training documentation, and analyzing whether it aligns with the needs of individuals supported to provide the highest quality services.
- Demonstrates the ability to coordinate with the planning team and direct support staff to ensure services are implemented effectively and meet the individual's needs.
- Assist in developing, reviewing, and updating the Individual Support Plan (ISPs) based on assessments, progress and identifying any needed changes.
- Provide guidance to staff regarding program implementation, documentation expectations, and best practices.
- Attend and participate in all ISPs annual updates and revisions.
- Assist with training to direct support staff on ISPs and outcomes, with ongoing coaching and retraining as needed.
- Review the drafted ISPs for accuracy and reports any discrepancies to the Supports Coordnator and the planning team immediately.
- Responsible for tracking and evaluating the individual’s progress toward goals to ensure they are effective and appropriate.
- Demonstrates ability to work effectively as part of a team.
- Knowledge of applicable regulations, including but not limited to Office of Licensing Standards, Community Care Participation Waiver, and Division of Developmental Disabilities.
- Ability to thrive in a fast-paced environment while managing and prioritizing multiple competing tasks.
- Demonstrates strong attention to detail, time management and organizational skills.
- Demonstrates excellent judgment with the ability to independently solve problems, make decisions with minimal or no direct supervision.
- Demonstrates ability to anticipate future needed changes, identify problem areas, and take effective actions.
- Provides excellent customer service, collaborates with the team, presentation skills, and communicates effectively both written and verbal skills.
- Performs all other duties as assigned.
QUALIFICATIONS :
- At least one year of experience working with individuals with intellectual or developmental disabilities (IDD).
- Bachelor's degree in human services and at least two (2) years of experience working directly with individuals with IDD.
- OR sixty (60) college credits in human services field with two (2) years of case management experience and an additional two (2) years of direct support with an IDD population, OR one (1) year IDD case management experience with five (5) years of direct support with an IDD population
- A Master's degree in a relevant field may be considered for the required years of experience.
- Non-profit human service experience highly preferred
- Must be able to perform CPR using agency-trained protocols
- Demonstrated intermediate or above experience with Microsoft Office applications, including Teams, Word, Excel, Outlook, and PowerPoint experience preferred
- Experienced using/knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred
- Proven ability to integrate diverse perspectives, foster innovation and accomplish objectives through managing, coaching, and developing a team
- Ability to manage processes, develop, and initiate actionable strategies that support the company's growth mission
- Excellent customer service skills with the ability to effectively communicate cross-functionally with all levels within an organization
- Team player with exceptional organizational skills that can work independently and be adaptable to changing schedules and demands
- High level of both written and verbal communication skills, capable of adapting style and tone according to the situation and audience
