The RN Supervisor directs nursing operations for the 3 11 shift and reports to the DON. The supervisor ensures safe, resident-centered care; oversees shift staff; makes sound clinical and operational decisions; and provides residents and families with empathetic, factual, and prompt communication.

Responsibilities
  • Supervise, assign, and support RNs, LPNs, CNAs, agency staff, and other assigned personnel based on resident acuity, staffing, competency, and scope of practice.
  • Conduct rounds; monitor completion of care, treatments, medications, documentation, and care-plan interventions; provide direct nursing care when needed.
  • Assess changes in condition, abnormal findings, incidents, and complaints; initiate interventions and promptly notify the practitioner, resident representative, DON, and others as required.
  • Use sound clinical judgment and problem-solving to address staffing, resident-care, and operational concerns; escalate significant or unresolved issues promptly.
  • Coordinate admissions, transfers, discharges, emergencies, deaths, and shift handoffs, ensuring complete documentation and continuity of care.
  • Communicate with residents and families respectfully and empathetically, providing timely factual updates, answering questions within the role, protecting privacy, and arranging follow-up.
  • Fall prevention: identify risks and hazards, verify individualized interventions and adequate supervision, respond to falls, complete assessments and notifications, and begin post-fall review and corrective action.
  • Implement infection-control practices, including hand hygiene, PPE, transmission-based precautions, isolation, surveillance, and prompt reporting of infections, exposures, or outbreaks; provide corrective education as needed.
  • Educate, coach, and evaluate staff regarding resident care, fall prevention, infection control, emergency procedures, resident rights, documentation, and identified competency needs.
  • Protect resident rights, dignity, and safety. Immediately protect residents and report suspected abuse, neglect, exploitation, mistreatment, injuries of unknown source, misappropriation, and other required events.
  • Respond to staffing shortages and emergencies; maintain appropriate coverage and notify leadership of staffing or safety risks.
  • Participate in monitoring of falls, infections, wounds, medication events, and other quality concerns.
  • Follow CMS/federal and New Jersey nursing-home requirements, the New Jersey Nurse Practice Act, professional standards, and facility policies. Perform other related duties consistent with licensure and competence.

Required Qualifications

  • Current, active, unencumbered RN license authorizing practice in New Jersey.
  • Graduate of an approved registered nursing program; current CPR/BLS certification.
  • Minimum two years of nursing experience; long-term care, skilled nursing, subacute, rehabilitation, or geriatric experience strongly preferred.
  • Prior charge nurse or supervisory experience preferred; demonstrated competency in assessment, delegation, emergency response, documentation, staff oversight, and resident/family communication required.
  • Working knowledge of CMS long-term-care requirements, New Jersey nursing-home regulations, fall prevention, infection control, resident rights, incident management, and quality improvement.
  • Ability to use the electronic health record and satisfy applicable health screening, background, exclusion, and employment-eligibility requirements.

Physical and Environmental Demands

The following are typical demands of the position. Reasonable accommodations will be considered for qualified individuals.
  • Frequently stand and walk; bend, reach, turn, push, pull, grasp, and perform clinical and computer tasks; occasionally sit for documentation.
  • Assist with resident repositioning, transfers, and mobility using approved equipment and sufficient staff. Lift/carry up to 25 pounds independently; use equipment or assistance for greater loads in accordance with safe-resident-handling practices.
  • Possess visual, auditory, tactile, and communication abilities sufficient to assess residents, read records and labels, hear alarms, communicate in emergencies, and safely perform assigned duties with or without reasonable accommodation.
  • Work around blood, body fluids, infectious diseases, cleaning agents, medications, odors, noise, and stressful situations; wear required PPE and follow exposure-control procedures.
  • Work the full 3:00 p.m. 11:00 p.m. shift and rotating weekends, holidays, overtime, emergency coverage, or schedule changes as assigned; maintain reliable attendance and remain until appropriately relieved.
