Job Title: Hybrid RN Case Manager
Location: Remote with required in-person home visits (NJ)
Coverage Area: Monmouth, Morris, Somerset, Union, Passaic, Bergen, and Essex Counties

Schedule & Compensation:

• Monday – Friday, 8:00 AM – 5:00 PM
• 1–2 days remote work per week
• Pay: $42.00 – $44.00 per hour, Weekly pay

Position Overview:
We are seeking a dedicated and compassionate Hybrid RN Case Manager to support members through comprehensive care coordination. This role blends remote work with in-person home visits, allowing you to make a direct impact on patient outcomes while maintaining flexibility in your schedule.

Key Responsibilities For Registered Nurse Case Manager:

• Assess, plan, implement, and coordinate case management activities to support member health and wellness
• Evaluate medical needs, benefit eligibility, and facilitate access to appropriate programs and services
• Apply clinical judgment to address complex health conditions, risk factors, and social determinants of health
• Conduct holistic assessments, considering co-morbidities and multiple diagnoses affecting overall functionality
• Review prior claims and medical history to inform current care planning
• Identify barriers to care and develop strategies to improve outcomes
• Coordinate referrals to clinical and community resources as needed
• Collaborate with interdisciplinary teams and participate in case conferences
• Maintain accurate documentation in compliance with regulatory and company standards
• Engage members through effective communication and interviewing techniques

Qualifications For Registered Nurse Case Manager:

• Active, unencumbered Registered Nurse (RN) license (New Jersey)
• Minimum of 2 years of RN experience required
• At least 2 years of home health or case management experience required
• Strong clinical assessment and care coordination skills
• Ability to work independently and manage a hybrid schedule
