Aging Advisors is a leading geriatric care management practice providing personalized care management services and solutions for older adults and their families throughout Mercer County NJ and surrounding areas. We help clients navigate complex medical, cognitive, emotional, and long-term care needs while promoting safety, independence, dignity, and quality of life.
Our team collaborates with physicians, hospitals, rehabilitation centers, senior living communities, home care agencies, attorneys, financial professionals, and other community partners to coordinate care, advocate for older adults, and provide expert guidance through every stage of aging.
Position Summary
Aging Advisors is seeking an experienced Care Manager to join our growing team. The Care Manager will provide comprehensive assessments, develop individualized care plans, coordinate services, advocate for clients, and serve as a trusted resource for older adults and their families.
This position requires exceptional communication, strong critical thinking skills, compassion, and the ability to independently manage complex client situations while collaborating with an experienced interdisciplinary team.
Essential Responsibilities
- Conduct comprehensive in-home, facility, hospital, rehabilitation, and virtual assessments.
- Develop, implement, and monitor individualized care plans based on each client's goals and needs.
- Identify medical, cognitive, emotional, social, environmental, and safety concerns.
- Educate clients and families regarding care options, available resources, and long-term planning.
- Coordinate healthcare services, home care, rehabilitation, behavioral health, hospice, senior living, and community resources.
- Accompany clients to medical appointments and advocate during hospitalizations and care transitions.
- Communicate regularly with physicians, healthcare providers, facility staff, caregivers, attorneys, financial professionals, and family members.
- Assist clients and families with transitions between home, hospitals, rehabilitation centers, assisted living, memory care, and skilled nursing facilities.
- Conduct family meetings to facilitate communication, education, and collaborative decision-making.
- Serve as a local advocate and liaison for long-distance family members.
- Arrange and monitor caregivers, transportation, medical appointments, and other support services.
- Recommend meaningful social, recreational, educational, and community activities that enhance quality of life.
- Provide crisis intervention and respond to changing client needs.
- Transport clients when necessary.
- Maintain accurate and timely documentation using the agency's electronic care management system.
- Participate in team meetings, case consultations, and ongoing supervision.
- Collaborate with Aging Advisors team members to ensure exceptional client care.
- Perform other duties as assigned.
Qualifications
- Master's degree in Social Work, Gerontology, Psychology, Counseling, Human Services, Occupational Therapy, or another related healthcare field or LPN/RN degree.
- Minimum of three years of experience working with older adults and family caregivers.
- Experience in geriatric care management, case management, home health, hospice, hospital discharge planning, rehabilitation, behavioral health, or senior living strongly preferred.
- Experience supporting individuals living with dementia, chronic illness, mental health conditions, or other complex care needs.
- Strong assessment, care planning, problem-solving, and crisis management skills.
- Excellent written, verbal, and interpersonal communication skills.
- Highly organized with the ability to prioritize and manage multiple clients.
- Ability to work independently while collaborating effectively within a team.
- Proficient with electronic documentation and Microsoft Office.
- Valid driver's license, reliable transportation, and automobile insurance required. Supplemental Auto Insurance provided.
Preferred Qualifications
- Gerontology, RN, LSW, LCSW, LPC, OT, LPN or other applicable professional license.
- Care Manager Certified (CMC) or Certified Case Manager (CCM).
- Certified Dementia Practitioner (CDP).
