This company is a premiere Medicare/Medicaid-certified provider of home health, hospice, and community-based services. We’ve been serving families and individuals with the most advanced, comprehensive clinical home care in NJ since 1912.

We are currently looking for a Staff Nurse to join our team. This position would be responsible for providing skilled nursing care to homecare patients.

Mercer County- Homecare Nurses

Hours: 8:30-4:30 Mon-Fri and only 1 weekend per month

Responsibilities:

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Assesses and evaluates the needs of patients with multiple disease processes and co morbidities and develops an appropriate plan of care
• Documents all changes in patient’s condition and updates care plans and outcomes
• Identifies teaching needs and plans appropriate strategies for individuals and families
• Monitors quality and appropriateness of services provided
• Establishes and maintains professional working relationship with health care providers and other members of the disciplinary teams in providing patient and family care
• Adheres to agency infection control and safety policies
• Adheres to the organization’s policy in regards to absenteeism and appearance
• Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this positionQualifications
• RN with Bachelor’s degree in Nursing preferred
• 1+ year of supervised clinical experience as a registered nurse
• Excellent verbal and written communication skills
• Strong computer skills
• Excellent organizational and interpersonal skills
• Car and valid driver’s license

Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact

Experience:

• 1+ year of supervised clinical experience as an RN: 1 year (Required)

Licenses or certifications:

• Driver's License (Required)
• Registered Nurse (RN) (Required)
