Haddon Home Care is launching an innovative Care Navigation and Case Management program, and we're looking for a passionate Social Worker to help us bring it to life.

About the Role:
As a Social Worker in our Care Navigation and Case Management program, you’ll provide expert care navigation and support to seniors and their families—while also serving as a key liaison between our agency and local hospitals, rehabilitation centers, and physician offices. This role combines traditional social work with community engagement, making it ideal for someone who enjoys both client-focused care and relationship-building.

Responsibilities Include:

• Conducting comprehensive assessments and developing individualized care plans
• Coordinating resources and support for seniors navigating complex health and social needs
• Collaborating with families, providers, and our in-house care team to ensure aligned care
• Marketing and promoting the Case Management program to healthcare facilities and medical professionals
• Representing Haddon Home Care at events, meetings, and outreach visits alongside our Director of Business Development & Community Relations
• Maintaining detailed documentation and providing ongoing support and advocacy for clients
• In-person and virtual visits with clients and their families on an ongoing basis

What We’re Looking For:

• Licensed Social Worker (LSW or LCSW) in the State of New Jersey
• Prior experience in case management, healthcare navigation, or senior services required
• Excellent communication and interpersonal skills
• Comfortable with both field-based and in-office work
• Valid driver’s license and reliable vehicle required
• Organized, compassionate, and a true team player
