Job Overview:
Facilitates the coordination of services for developmentally delayed children who qualify for Early Intervention. Assists child|family in development of the Individualized Family Service Plan. Provides supportive counseling and crisis intervention to child|family. Advocates for children on state and local level.
Qualifications:
Required:
- One or more of the following acceptable:
- Bachelor's Degree in Psychology or Social Work
- Teacher of the Handicapped
- Master's degree in social work (LSW) from an accredited program
- A NJ licensed nurse with a bachelor’s degree in Nursing (BSN)
- One-year experience in public health or with children who are chronically ill or disabled
- One year of applicable exerience with birth to 5 years old population
- Ability to work independently with a demanding client caseload
- Ability to provide supportive counseling and crisis intervention
- Exhibits empathy and cultural/ethnic sensitivity to a diverse client population
- Effective problem-solving abilities
Preferred:
- Minimum of two years of work as a Service Coordinator
- Active License in the state of NJ
Scheduling Requirements:
- Full Time
Essential Functions:
- Maintains a caseload of approximately 60 to 75 children and completes all mandated work
- A part of the hiring process for all new Service Coordinators and works jointly with the Director/ Unit Coordinator, Program Coordinator and Senior Service Coordinator to orientate new Service Coordinators
- In charge of monthly tracking of Annual/Exit BDI lists, Works with the Hudson County Targeted Evaluation Teams and Service Coordinators to ensure adherence to NJEIS Policy and best practices
- Establishes working relationship with children who are developmentally delayed and their families
- Coordinates the scheduling of assessments in accordance with guidelines established by Early Intervention/DOH
- Assessment of children's service needs in the following areas: social|emotional. developmental, familial medical, cultural, economic, educational, etc.
- Development and review of the Individualized Family Services Plan (IFSP) for all eligible Part C children. Ensures all appropriate notification is provided
- Facilitates the coordination of services for Part C eligible children|families through linkage to service providers and EI programs
- Promoting and facilitating communication among service providers working with the child|family. Advocating for appropriate services
- Supportive counseling and crisis intervention for Part C eligible children|families
- Assist families with transition planning to preschool, special education or other services considered appropriate at age three
- Ability to manage electronic files including data entry into EIMS/DOH database
- Facilitate functioning of the Part C Unit in the absence of the Unit Coordinator
- Participates in the development, evaluation and revision of the Unit ’ s policies and procedures
- Works jointly with Director/Program Coordinator to meet department’s internal goals and strategic metrics
- Participates in all department, hospital and outside agency/institution committees as assigned/delegated
- Prepares/maintains unit readiness for inspections, surveys, ensures program compliance with all standards
- May travel by car throughout work day to attend meetings and conduct home visits
- As necessary, oversees twice weekly broadcast of eligible children and related duties
