Shift:
Full-Time, Monday – Friday, 8:30 AM – 4:30 PM (Must be flexible to work outside normal business hours if needed)

About Us and Our Program:

Easterseals, a national nonprofit, has supported individuals with disabilities for over 50 years. Its Behavioral Health Home program offers care coordination services, ensuring individuals diagnosed with a mental illness get connected to the health and community services they want and need.

What You’ll Do:

The Care Coordinator is responsible for supporting individuals with complex needs through integrated care planning, service coordination, and advocacy. They will work across disciplines to ensure that clients receive seamless access to the services they need to thrive—whether medical, behavioral, or social.

Key responsibilities include:

· Conduct comprehensive intake assessments and develop individualized assessments to ensure consumers are linked to all applicable services.

· Serve as the central point of coordination among physical health, behavioral health, and social service providers.

· Facilitate interdisciplinary team meetings to monitor client progress and adjust care plans accordingly.

· Identify and address barriers to care, including housing, food security, transportation, and access to behavioral health services.

· Maintain accurate and timely documentation in compliance with program and licensing standards.

· Market existing services to clients.

· Partner and work closely with the intake coordinator

· Engage in case conferencing, advocacy, and crisis intervention as needed.

· Monitor client outcomes and satisfaction to ensure a high standard of care and continuous improvement.

· Travel across different counties may be required.

Who You Are:

· Education: Master’s degree in Social Work, Counseling, Psychology, or a related field required.

· Experience: At least 2 years of experience providing case management or care coordination services, preferably in a health or human services setting.

· Requirements: Active clinical license in the State of New Jersey: Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW).

· Skills: Possess strong knowledge of behavioral health systems, Medicaid services, and community-based resources in New Jersey, with proficiency in electronic health records and documentation practices.

· Attributes: Excellent communication, organizational, and problem-solving skills, along with a demonstrated commitment to trauma-informed, culturally competent, and person-centered care.
