C ompensation: $130,000 - $145,000 Annual Salary

Position Description:

We are looking for a dedicated Nurse Practitioner ( NP ) in Lafayette NJ for our Skilled Nursing Facility ( SNF ) for bedside patient care. This is a full time, in person position, Monday - Friday 9am - 5pm.

Qualifications:

Valid Nurse Practitioner ( NP ) license in the state of New Jersey & New York

1 -2 years experience of Nurse Practitioner ( NP ) experience
Active DEA License
Nursing home and ICU experience preferred

Responsibilities:

Upon admission to the facility, examine residents and their medical records.
Check ups, management of chronic conditions and medications, transitional care management, discharge planning and family engagement.
