- Clinical Care: Conduct comprehensive patient assessments, diagnose conditions, and develop and manage complex care plans.
- Documentation: Accurately document all patient encounters and treatments using Electronic Health Records (EHR).
- Collaboration: Work seamlessly with physicians, nursing staff, and other healthcare professionals.
- Communication: Serve as a key communicator with patients’ families and support systems.
- Participation: Attend facility meetings and actively contribute to care planning discussions.
- Standards: Maintain the highest level of professionalism and uphold HudsonView Center's commitment to clinical excellence.
- Education: Master’s or Doctorate degree in Nursing from an accredited program.
- Licensure: Active New Jersey APN and RN Licenses.
- Certifications: Active DEA, CDS, and BLS certifications.
- Experience: Minimum 2 years of Nurse Practitioner experience.
- Expertise: Proven experience managing elderly patients and complex medical conditions.
- Tech: Proficiency in electronic health records (EHR).
- Experience in post-acute care environments.
- Strong procedural skills, including: IV insertion, G-tube replacements, wound care, and suture/staple removal.
