Overview:
As a Physical Therapist, with Vitality Health Care you will be an integral part of our team, providing high-quality rehabilitative care to patients within the comfort of the patient's own homes. You will work closely with patients to assess their physical conditions, develop personalized treatment plans, and implement therapeutic interventions to improve mobility, alleviate pain, and enhance overall quality of life. Your expertise and compassionate approach will empower patients to regain independence and achieve their rehabilitation goals within the familiar and supportive environment of their homes. We provide 1 on 1 treatment session's in the patient's home, through Medicare Part B.


Responsibilities:

  1. Conduct comprehensive assessments of patients' physical capabilities, medical histories, and functional limitations to establish baseline data and formulate individualized treatment plans.
  2. Develop and implement evidence-based physical therapy interventions aimed at improving patients' mobility, strength, balance, coordination, and range of motion.
  3. Educate patients and their caregivers on proper techniques for performing therapeutic exercises, using assistive devices, and implementing strategies to prevent injuries and promote optimal physical function.
  4. Monitor patients' progress, reassess goals, and modify treatment plans as necessary to ensure optimal outcomes and continuity of care.
  5. Collaborate with interdisciplinary healthcare team members, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate patient care and promote holistic rehabilitation.
  6. Maintain accurate and up-to-date documentation of patient evaluations, treatment sessions, progress notes, and discharge summaries in compliance with regulatory standards and agency policies.
  7. Provide emotional support and encouragement to patients and their families, fostering a trusting and therapeutic relationship conducive to effective rehabilitation.
  8. Stay abreast of advancements in physical therapy techniques, technologies, and research findings, and integrate new knowledge into clinical practice to enhance the quality of care provided.
  9. Adhere to ethical principles and professional standards of conduct, ensuring confidentiality, respect for patient autonomy, and adherence to legal and ethical guidelines.
  10. Participate in ongoing professional development activities, such as continuing education courses, seminars, and workshops, to enhance clinical skills, maintain licensure, and pursue specialty certifications as appropriate.

Qualifications:

  1. Graduate of an accredited physical therapy program with a Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) degree.
  2. Valid state licensure as a Physical Therapist, with current CPR certification.
  3. Prior experience in home health, geriatrics, or rehabilitative care preferred.
  4. Strong clinical assessment and critical thinking skills, with the ability to develop and implement individualized treatment plans based on thorough evaluations.
  5. Excellent interpersonal and communication skills, with the ability to establish rapport with patients, caregivers, and interdisciplinary team members.
  6. Compassionate, patient-centered approach to care, with a commitment to empowering patients to achieve their rehabilitation goals.
  7. Ability to work independently and manage time effectively, prioritizing tasks and adapting to changing patient needs in a dynamic home health environment.
  8. Proficiency in documentation software and electronic health records (EHR) systems.
  9. Willingness to travel to patients' homes within designated service areas and maintain a valid driver's license and reliable transportation.
  10. Commitment to upholding ethical standards and complying with regulatory requirements governing physical therapy practice.
