This contract to hire position involves working in community or small facility settings within Passaic County, focusing on health maintenance, disease prevention, and case management.

Key Responsibilities of the RN - Home Health - Travel

• Assess the health of individuals and implement care plans
• Ensure health maintenance and disease prevention
• Provide case management
• Supervise care plans and staff members
• Observe, assess, plan, implement, and evaluate nursing support
• Coordinate with other health professionals/providers
• Ensure proper medication management
• Maintain communication about patient health progress
• Keep detailed nursing documentation
• Supervise licensed practical/vocational nurses
• Schedule and manage health-related appointments
• Provide staff training on health and safety topics

Requirements of the RN - Home Health - Travel

• RN License
• CPR Certification
• Experience in wound care, foley catheter, IV Infusion, PICC/midline management
• Proficiency in HCHB/Point Care
