π Job Responsibilities:
As a HomeSafe Therapist/Clinician, you will play a vital role in supporting families dealing with stressors like homelessness, substance abuse, mental health concerns, and child separation due to these stressors. Your responsibilities include:
π Programmatic and Service Model Mastery:
π Prompt Client Outreach: Respond promptly to client requests for phone screens and intake assignments.
π Accurate Assessment: Assess clients' presentations, needs, and strengths accurately.
π Timely Intake Assessments: Complete intake assessments promptly.
π€ Engagement, Planning, and Contracting:
π Collaborative Planning: Develop a care plan in partnership with clients, incorporating their needs and priorities.
β³ Progress Monitoring: Regularly evaluate progress and renegotiate goals for growth.
π Ensure Follow-Ups: Follow up on missed or canceled services.
π Ongoing Reassessment: Re-negotiate planning at regular intervals to address evolving client needs.
π Clinical Excellence:
π¦ Effective Intervention: Utilize appropriate interventions, both in non-crisis and crisis situations.
β€ Empathy and Understanding: Create a judgment-free, empathetic space for clients, respecting their feelings and personal presentation.
π― Tailored Approaches: Adapt your approach to meet client needs, whether it's individual, family, group, or crisis intervention.
π Medication Management: Consult with psychiatrists, monitor medication regimens, and attend to medication appointments.
π€Ή Case Management: Provide case management services as needed.
π€ Collaborative Care: Advocate, coordinate, and maintain contact with other providers involved in clients' care.
π Trauma-Informed Care: Incorporate trauma-informed care models.
πͺ Discharge and Termination:
π Goal Achievement: Identify when therapeutic goals have been met and work toward appropriate discharge.
π£ Client Preparation: Discuss progress with clients and facilitate timely discharge planning.
π Transfer Assistance: Facilitate transfers/referrals for further services when appropriate.
π Supervisory Consultation: Consult with clinical and program supervisors before discharge.
π Paperwork and Planning: Complete termination paperwork and discharge planning promptly.
π Documentation, Meetings, and Productivity:
π Accurate Documentation: Complete documentation following agency and program guidelines.
π€ Collaborative Meetings: Facilitate meetings and collaborate with referral sources as needed.
ποΈ Attend Team Meetings: Actively participate in internal team meetings and case presentations.
π Meet or Exceed Productivity Targets: Maintain or exceed program productivity requirements.
π Transportation Responsibilities:
- π Vehicle Safety: Maintain a professional and safe environment in agency vehicles.
- π‘οΈ Child Safety: Ensure the safety of children and adults during transportation.
- π§½ Vehicle Maintenance: Monitor and document vehicle maintenance regularly.
- ποΈ Log Maintenance: Keep required vehicle logs and maintain records.
- π¦ Transportation Coordination: Prepare for pick-up and drop-off of clients for program services.
- π£οΈ Clear Communication: Communicate effectively with parents, caregivers, and children during transportation.
π Qualifications:
To excel in this role, you should possess:
- π Valid NJ State Driver's License in Good Standing.
- π Master's Degree in Social Work and a NJ State License such as LSW, LPC, orΒ LMFT.
- π₯ Two years' experience working with children and families, especially in child welfare.
- πͺ Experience with diverse populations in an urban setting.
- π§ Knowledge of mental health, various treatment modalities, and community resources.
- π¨ Crisis Intervention Skills: Ability to apply appropriate interventions in crisis and non-crisis situations.
- π Trauma-Informed Care Knowledge: Familiarity with interventions for families experiencing abuse or trauma.
- π’ Organizational Skills: Solid organizational and time management skills.
- π Team Player: Ability to work well both independently and as part of a team.
- πΊ Navigation Skills: Ability to support clients in navigating multiple care systems.

Salary Range: $62,000 - $65,000
Vacation & Benefits:- Vacation: As a full-time employee working generally 35 hours per week, you will be eligible to participate in the Family Connections group benefit plans. Benefits include medical, dental, life insurance, long term disability, retirement plans, and additional supplementary benefits. Yes, you can elect to not enroll into these benefits. You will also be eligible to accrue vacation, personal, and sick days in addition to 12 paid holidays, and summer wellness Fridays.
