Join the LOES (Loving Our Elders and Seniors) Team as a Registered Nurse (RN) - Per Diem
As a Registered Nurse with LOES, you will play a vital role in ensuring exceptional care for our clients. Your responsibilities will include: Initial Assessments: Completing comprehensive evaluations for new clients to determine care needs.
Routine Home Visits: Monitoring and updating client care plans during monthly visits.
PRN (As-Needed) Visits: Responding to client needs with timely assessments.
Post-Hospital Assessments: Ensuring smooth transitions from hospital to home care.
Recertification Assessments: Reassessing care needs every six months to ensure optimal support.
Supervision of Home Health Aides: Overseeing aides during visits to ensure compliance with care plans and quality standards Skills
Strong clinical experience in various settings such as hospitals, nursing homes, and primary care.
Knowledge of geriatric and long-term care best practices.
Excellent communication and interpersonal skills to collaborate with clients, families, and healthcare teams.
The ability to work independently while managing priorities effectively.

- At least 2 years of experience as a Registered Nurse.
- Current BLS and CPR certifications .
