When facing a mental health and/or substance abuse crisis, it can be difficult to know where to turn. CREST can help you overcome your crisis and connect you to the ongoing services you need. CREST provides rapid access to short-term, recovery-oriented crisis services to address both crisis intervention and stabilization for adults experiencing a mental health and/or substance abuse crisis. Our crisis services help people as they navigate their crisis, overcome obstacles, and plan their ongoing mental health needs

Position Title: Case Manager
Position Type: Full Time; 40 Hours Weekly

Responsibilities:

• Participate in a multi-disciplinary team
• Engage all consumers in treatment planning, WRAP planning and all areas of service delivery
• Assist consumers with activities of daily living
• Provide direct assistance in obtaining basic needs for consumers
• Provide on-going assessment of client's emotional well being, clinical symptomology, behavioral changes and medication compliance

Requirements

• Bachelor's Degree in a Human Services field
• Experience with Crisis Intervention preferred
