Overview
Physical Therapist – Travel assignment in Fair Lawn, New Jersey. Start date 04/20/2026 for 13 weeks in a skilled nursing facility setting. Weekly pay range $2,428-$2,471 per week. Guaranteed hours 35 per week. Shift 5x8 Days (08:00–16:30). This is a travel, temporary assignment.

Responsibilities
- Conduct initial physical therapy evaluations and periodic re-evaluations for residents in a skilled nursing setting.
- Develop and implement individualized treatment plans addressing mobility, strength, balance, gait, and activities of daily living.
- Administer therapeutic interventions including exercise programs, gait training, manual therapy, and assistive device training as prescribed.
- Document assessments, treatment plans, progress notes, and discharge planning in the patient record; monitor progress and adjust goals accordingly.
- Collaborate with an interdisciplinary team (PT, OT, SLP, nursing, physicians) to coordinate care and support smooth transitions between care phases.
- Ensure patient safety, infection control, and adherence to regulatory and facility protocols during all interventions.

Requirements
- Active physical therapy license in good standing (or eligibility for licensure) in New Jersey.
- Degree from an accredited physical therapy program (DPT preferred; PT accepted with appropriate credentials).
- Ability to work in a skilled nursing/geriatric rehabilitation setting.
- Effective communication and documentation skills; proficiency with required clinical documentation systems.
- Availability to start on 04/20/2026 and commit to a 13-week assignment.
- Willingness to travel to the assignment location and perform duties consistent with a travel healthcare role.
