Job Summary:
The Home Care Sales Liaison is responsible for generating referrals and building strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources. This individual acts as the face of the agency in the community and plays a vital role in expanding the agency’s reach and visibility, promoting services, and ensuring seamless transitions of care for new clients.
Key Responsibilities:
Develop and maintain relationships with referral sources, including hospitals, physicians, discharge planners, case managers, and social workers
Represent the agency at networking events, health fairs, and community functions to build brand awareness
Educate referral sources on home care services, including personal care, companion care, skilled nursing, and therapy services
Follow up on leads and referrals promptly and ensure smooth onboarding of new clients
Track referral activity and maintain accurate records in CRM systems
Collaborate with the clinical and scheduling teams to ensure client needs are met and expectations are exceeded
Meet or exceed monthly referral and admission targets
Serve as a resource to families and potential clients, helping them navigate options and eligibility for services
Monitor market trends and competitor activity to adjust sales strategies accordingly
