Job Overview:
The Social Worker provides emotional and administrative support to patients, either as a group or individual , through group process and psychotherapy. All methods of practice shall conform to written hospital policies and procedures and be carried out to the highest achievable level of competence in a manner demonstrating concern and consideration. The Licensed Social Worker - LSW will provide treatment, social work services and complex discharge planning needs. This position is an integral member of the interdisciplinary care team, working alongside physicians, nurses, chaplains, and case managers to ensure that every patient receives compassionate, patient centered , and culturally sensitive care. The Social Worker helps patients and families navigate complex medical decisions, understand care options, and access community resources while honoring their values, goals, and dignity.
Qualifications:
Required:
- Graduation from an accredited graduate school of social work with an MSW master's in social work degree
- State licensure is required by the NJ State board of Social Work Examiners
- Licensed clinical social worker LCSW or licensed social worker LSW
- 2-3 years’ experience in a hospital setting preferred
- Experience in healthcare, hospice, palliative care, oncology, or related fields.
- Strong communication, counseling, and crisis intervention skills.
Preferred:
- LCSW strongly preferred.
- Certification in Hospice and Palliative Social Work (CHP SW) or willingness to obtain.
- Experience working with diverse, urban communities.
Scheduling Requirements
- 40 hours/week

Essential Functions:
- Responsibilities include but are not limited to: Group and individual patient supervision, facilitation of educational group activities, assisting with ADL's (i.e., Feeding, bathing, toileting, ambulating, etc.) & taking and recording vital signs.
- Conduct biopsychosocial for all patients, engage families and or community support services to assess patients' psychological, social, environmental and financial status and meet identified needs
- Develop treatment plans with patients and the our interdisciplinary team
- Participate in interdisciplinary treatment team planning rounds to assess discharge planning needs, identify problems and communicate on-going interventions
- Educate and engage patients and or families about treatment, discharge planning, patient needs, as well as availability and eligibility criteria for programs services benefits to meet those needs
- Creatively negotiate, coordinate, and advocate community based service to assure the highest level of needed supports, continuation of care and wrap around services
- Document in the Medical Record all assessments, reassessments and interventions to enhance multidisciplinary communications, as well as, to meet all regulatory and reimbursement standards
- Attends appropriate Social Work conferences/workshops to maintain CEUs and license specific requirements
Key Responsibilities
Patient & Family Support
- Provide psychosocial assessments for patients with serious or life limiting illnesses.
- Offer counseling, crisis intervention, and emotional support to patients and families.
- Facilitate family meetings to clarify goals of care, treatment preferences, and advance care planning.
- Support patients and families through grief, anticipatory loss, and end of life transitions.
Care Coordination & Advocacy
- Collaborate with the interdisciplinary palliative care team to develop individualized care plans.
- Advocate for patient rights, preferences, and culturally appropriate care.
- Coordinate referrals to community resources, hospice programs, mental health services, and financial or housing support as needed.
- Assist with discharge planning and continuity of care.
Education & Communication
- Educate patients and families about palliative care services, advance directives, and available support systems.
- Communicate clearly and compassionately with diverse populations, respecting cultural, spiritual, and linguistic needs.
- Participate in staff education, team meetings, and quality improvement initiatives.
Documentation & Compliance
- Maintain accurate, timely documentation in the electronic medical record.
- Ensure compliance with hospital policies, state regulations, and ethical standards of social work practice.
- Participate in data collection and program development to strengthen program outcome measures.
