Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor and observe patient health status, including vital signs and overall well-being, while accurately documenting medical information in Electronic Health Record (EHR) systems
- Administer medications following medical instructions and ensure proper medication documentation in compliance with home healthcare regulations and HIPAA privacy standards
- Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care strategies and behavior management
- Support patients with complex needs such as catheter care, hospice care, end-of-life assistance, and managing medical devices or equipment
- Prepare nutritious meals tailored to dietary restrictions and assist with feeding as needed; perform light housekeeping tasks like cleaning and laundry to maintain a safe environment
- Use medical terminology confidently to communicate effectively with healthcare teams and document patient progress accurately
