Job Summary
ND Global Consulting Services, Inc. is seeking a qualified Speech-Language Pathologist / SLP for a travel healthcare assignment. The SLP will provide speech therapy services to patients in home care and facility-based settings, including assessment, treatment planning, patient and family education, documentation, and coordination with physicians, nurses, and other clinical team members.
The ideal candidate will have experience in acute care, subacute care, home care, or a comparable clinical setting and must be able to travel to patient homes and facilities as needed.
Responsibilities
- Perform patient assessments and determine appropriateness for services.
- Assess patient communication, swallowing, and cognitive abilities as clinically indicated.
- Collaborate with patients, families, physicians, nurses, and other clinical disciplines to develop and implement individualized plans of care.
- Provide speech therapy services in accordance with physician orders, agency policies, and applicable clinical standards.
- Instruct patients and families in communication strategies, safe swallowing techniques, cognitive training, and therapeutic exercises as indicated.
- Develop, implement, and revise treatment programs to restore function and help patients achieve maximum functional status.
- Monitor patient progress toward goals and adjust treatment plans as needed within physician orders.
- Provide discharge planning and referrals to appropriate resources when needed.
- Maintain a safe environment for patients and coworkers.
- Assess the patient’s social environment and provide education or intervention as needed.
- Identify potential clinical risks and adapt treatment plans as appropriate.
- Follow universal infection-control practices and agency safety procedures.
- Complete timely and accurate documentation in accordance with agency policies and regulatory requirements.
- Demonstrate knowledge of home care regulations and insurance reimbursement guidelines.
- Participate in case management, interdisciplinary communication, team meetings, and care coordination.
- Follow scheduling procedures and maintain required visit productivity standards.
- Travel to patient homes and facilities as assigned.
- Participate in orientation, cross-training, and other duties as assigned.
Requirements
- Graduate of an accredited speech and language program with a minimum of a master’s degree.
- Current New Jersey Speech-Language Pathologist / Speech Therapist license required.
- Minimum of one year of experience in an acute care, subacute care, home care, or comparable clinical setting.
- Strong communication, documentation, and customer service skills.
- Ability to work independently and manage patient visits in home and facility settings.
- Knowledge of home care standards, patient safety, infection control, and interdisciplinary care coordination.
- Ability to travel to patient homes and facilities as required.
- Use of a personal vehicle is required for home visits.
- Must meet all onboarding, credentialing, background check, and compliance requirements before assignment.
Preferred Experience
- Home health or home care experience.
- Experience with swallowing, communication, cognitive training, and functional rehabilitation.
- Experience coordinating care with physicians, nurses, families, and interdisciplinary teams.
- Prior travel healthcare or contract assignment experience.
Work Environment
This position may require travel to patient homes, healthcare facilities, and other assigned service locations. The SLP must be comfortable working independently in the field while maintaining communication with the clinical team and NDGCS management.
