Social Worker
- Complete and maintain comprehensive psychosocial assessments for all residents upon admission, quarterly, annually, and with significant changes
- Develop and implement individualized care plans that address psychosocial needs, and update them as necessary
- Provide emotional support, counseling, and crisis intervention for residents and families
- Participate in interdisciplinary care plan meetings and document social service input
- Assist residents and families with discharge planning and referrals to community resources
- Respond to and resolve resident grievances and concerns in accordance with resident rights
- Maintain timely, accurate, and regulatory-compliant documentation in resident medical records
- Ensure compliance with federal and state regulations, including CMS and OBRA guidelines
- Promote and uphold the rights and dignity of each resident
- Bachelor’s Degree in Social Work (BSW) from an accredited institution
- Active New Jersey certification or license to practice as a Social Worker (e.g., CSW, LSW)
- Minimum of 1 year of experience in a skilled nursing facility or long-term care setting
- Strong working knowledge of long-term care regulations, including MDS, care planning, and documentation standards
- Excellent interpersonal, communication, and conflict-resolution skills
- Ability to manage time, prioritize tasks, and work collaboratively in a multidisciplinary team
