Regional Hospital Liaison
AristaCare Health Services
AristaCare is seeking a Regional Hospital Liaison to develop and strengthen relationships with hospitals and referral sources in Union County, NJ . This role is responsible for driving admissions, coordinating patient referrals, and promoting AristaCare’s skilled nursing and rehabilitation services.
Key Responsibilities:
- Build and maintain relationships with hospital case managers, discharge planners, and physicians
- Coordinate patient referrals and facilitate smooth admissions to AristaCare facilities
- Conduct on-site hospital visits and represent AristaCare services
- Track referral activity and support census growth initiatives
- Collaborate with facility Admissions, Clinical, and Marketing teams
- Healthcare marketing, admissions, or liaison experience preferred (SNF or hospital experience a plus)
- Strong communication and relationship-building skills
- Knowledge of Union County hospital market preferred
- Ability to travel locally within Union County
