Case Manager

- Location: Bergen County, NJ – 100% On-site, Field work/Home health settings
- Payrate: $56.14/hr + mileage
- Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM
- On-Call: Weekend on-call rotation required.
- Meetings: Weekly Case Conference every Wednesday at 8:45 AM.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
- Comprehensive Assessment: Perform detailed assessments of high-risk members to identify medical and non-medical needs.
- Care Planning: Collaborate with primary care providers to implement individualized care plans with specific interventions.
- Care Coordination: Act as the bridge between physicians, social workers, discharge planners, and claims professionals to ensure smooth transitions of care.
- Patient Advocacy: Protect patient privacy and confidentiality while coordinating support services like housing and transportation to ensure treatment compliance.
- Outcome Management: Maintain detailed records of clinical, functional, and fiscal outcomes.
- Preventative Care: Identify opportunities for health promotion and intervene quickly to prevent adverse patient occurrences.
- Education: Associate’s or Bachelor’s Degree in Nursing (or a related field).
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Licensure: Current, unencumbered state Registered Nurse (RN) license.
- Certification: Case Management certification is required.
- Proficiency with Microsoft Teams and general office technology.
- Preferred: Experience with Home Care Home Base (HCHB) , PointCare, or PointClickCare.
- Strong ability to analyze complex medical information and make rational, evidence-based decisions.
