Job Title: Licensed Master Social Worker

Location: Morristown, NJ 07960

Duration: 13 Weeks with possible ongoing extensions

Schedule: 8 AM- 4 PM - weekend requirement

Pay Range: $56 - $61/Hour

Job Description & Duties:

• Provides therapeutic interventions and social work services to support patients and families, ensuring continuity of care in hospital and community settings.
• Conducts psychosocial assessments through interviews and participates in multidisciplinary rounds, offering input on social contexts and care planning.
• Acts as a resource and advocate for patients in areas such as advanced care planning and social determinants of health.
• Offers emotional and psychosocial support during illness or life transitions, collaborating with healthcare teams for both in-hospital and post-discharge needs.
• Coordinates discharge planning for services like rehab, SNFs, hospice, or homelessness solutions; maintains strong ties with community resources.
• Maintains detailed documentation per hospital, state, and federal requirements, while evaluating program effectiveness and ensuring confidentiality.
• Possesses strong communication, conflict resolution, and knowledge of regulatory standards; physical requirements include full mobility, hearing, and visual acuity.

Required Experience:

• Must have recent in-patient hospital experience & discharge planning experience.

Education:

• Master’s in social work

License:

• LMSW – New Jersey
