About the Role:

As a Home Health Aide , you will work as part of a multidisciplinary team to provide personalized care to patients who require assistance with daily living activities. Your responsibilities will include providing assistance with daily living activities such as bathing, dressing, grooming, and ambulation, as well as assisting with medication management and wound care.

Responsibilities:

• Provide assistance with daily living activities such as bathing, dressing, grooming, and ambulation
• Assist with medication management and wound care
• Monitor vital signs and report changes to the healthcare team
• Maintain accurate records of patient care and services provided
• Collaborate with patients, families, and healthcare professionals to ensure comprehensive care

Requirements:

• High School diploma or equivalent required
• 1 year of experience as a Certified Home Health Aide or related field
• Current certification as a Certified Home Health Aide (CHHA) or willingness to obtain
• Ability to work evenings, including weekends and holidays as needed
• Strong communication and interpersonal skills
• Compassionate and patient-centric approach to care
