Department: PATH-Housing Support Program
Salary: $55,000-$60,000
Location: Elizabeth, NJ
Work hours: Monday, Wednesday and Friday: 9am – 5pm; Tuesday and Thursday 10 to 6pm
Position Overview : The Peer Housing Manager initiates and manages the Bridgeway NJ FamilyCare Housing Services pilot program once the billing and IT systems and referral structure are in place. The Peer Housing Manager will initiate this service providing direct housing support beginning in Union County with capability of expansion to other counties in Central NJ in which Bridgeway provides housing and homeless services.
Essential Job Functions:
- Create and maintain a wellness and recovery-oriented environment by using person-first language, respectful documentation, and person-centered planning. All services provided will promote wellness and recovery outcomes with positive employment and educational outcomes for persons receiving services.
- Maintain a culture of compliance with internal and external policies, regulations, laws and high ethical standards.
- Conduct in-depth person served intake, assess housing barriers, and determine housing and service needs
- Develop a housing plan with persons served to resolve homelessness or risk of homelessness
- Assist person served with finding affordable housing
- Create and maintain relationships with landlords, serve as a liaison between property managers/landlords and person served
- Provide mediation and advocacy with landlords on persons served behalf to maintain housing.
- Support persons served accountability for achieving goals in their action plan
- Refer persons served to community resources to gain support for goals and follow-up (mental health services, substance abuse, employment, medical, etc.)
- Clinically appropriate disclosure of personal life experience to enhance engagement/validation with individuals
- Direct assistance to secure safe, clean, affordable housing, food and clothing, medical and dental services, and appropriate financial support.
- Linkage to entitlement benefits such as social security, rental assistance, and other public assistance (social services, transportation, legal advocacy, and representation)
- Rapid and flexible response to crisis, including but not limited to: accompanying the individual to the screening center/psychiatric emergency center and remaining with the person during the assessment process
- Assess persons served barriers to self-sufficiency and develop problem-solving skills with the person served.
- Provide best practice case management to singles and families, to include crisis intervention, ongoing assessments, goal setting, problem solving, advocating, agency and community resource utilization, and network building
- Utilizes evidenced based practices assigned which may include motivational interviewing, IDDT, CBT skills, IMR, Trauma Informed Care, housing first model and Career Development
- Technical assistance and education to members of the team in the area of lived experience.
Requirements
Education/Certifications:
• Bachelor’s degree preferred. A person who has lived experience may substitute demonstrated volunteer or paid experience working with individuals who are homeless in lieu of a Bachelor’s degree.
Experience:
• Person with lived experience is required.
Driver’s License, Vehicle:
• Valid driver’s license required
• No more than one moving violation within the past 12 months
• Vehicle required
