Role Overview
As a Continuous Care LPN , you will play a critical role in managing acute symptom crises at the bedside, ensuring the patient remains comfortable and supported in their preferred setting. The LPN works under the supervision and direction of the RN Case Manager and initiates the appropriate interventions in support of the patient and family. The LPN also follows a comprehensive and responsive plan of care.
Key Responsibilities
- Provide skilled, compassionate bedside care during periods of medical crisis or intense need
- Ensure and document care and interventions provided as well as appropriate use of medication, durable medical equipment and supplies.
- Provide accurate documentation as outlined in hospice, state and federal guidelines/standards. Documentation must be prepared and turned in according to these guidelines.
- Follows RN care plan appropriately. Report any changes in the patient's status to the RN case manager or Triage Nurse.
- Report necessary information to the Director of Clinical Operations, Regional Manager, RN case manager and other members of the IDT.
- Respond to issues as clinically appropriate.
- Work in conjunction with RN case manager.
- Provide clinical advice and emotional support to patients and their family members.
- Administers medications accurately, observing patient response, document medication and look for negative outcomes.
- Remain with patient providing care until shift relief arrives to continue care.
QUALIFICATIONS:
- NJ Nurse License required
- Minimum one (1) year of clinical experience (hospice, home care, med-surg or long-term care preferred)
- Understanding of hospice philosophy and the physical, emotional, and spiritual needs of terminally ill patients
- Strong problem solving and critical thinking skills
- Excellent Communication Skills
Reliable transportation and ability to travel to patient homes or facilities throughout designated counties
