Geriatric Care Manager - Licensed Social Worker or Registered Nurse
Mint Care Management at The Senior Company is seeking an experienced Licensed Social Worker (LSW/LCSW) or Registered Nurse (RN) to join our geriatric care management team serving older adults and their families throughout New Jersey.
This role focuses on providing high-touch, personalized support to aging clients and their families, many of whom live out of state and rely on us as their trusted local advocate. Our care managers help families navigate complex medical, social, and care planning decisions while ensuring clients receive the highest quality care and support.
Key Responsibilities
• Conduct comprehensive in-home assessments
• Develop and oversee individualized care plans
• Coordinate and monitor medical care, home care, and community services
• Accompany clients to medical appointments and advocate when needed
• Provide ongoing support, guidance, and education to families
• Assist with care transitions (hospital, rehab, assisted living, memory care)
• Maintain consistent communication with families and care teams
• Serve as a trusted liaison between families, providers, and caregivers
Qualifications
• New Jersey LSW, LCSW, or RN license required
• Excellent communication and relationship-building skills
• Excellent time management and organizational skills with the ability to manage multiple clients and priorities simultaneously
• Experience working with older adults, dementia, and complex medical needs
• Strong knowledge of healthcare systems, long-term care options, and community resources
• A genuine passion for helping older adults and supporting families
• Ability to work independently with a high level of professionalism and discretion
• Reliable transportation and willingness to travel throughout Northern New Jersey
• Certification as a Geriatric Care Manager (CCM, CMC, or equivalent) is highly desirable
Why This Role Is Unique
Geriatric Care Management offers a level of flexibility and autonomy that is rare in traditional healthcare roles. Care managers structure their day around client needs rather than a traditional 9-5 schedule. The work includes a mix of in-home visits, care coordination, family communication, advocacy, and problem-solving, making every day different.
Travel is a regular part of this role. Care managers spend a significant amount of time in the field visiting clients, attending appointments, and coordinating care throughout Northern New Jersey.
This is a relationship-based role. Successful care managers enjoy getting to know their clients, earning the trust of families, and serving as a consistent source of support through both everyday needs and times of crisis.
While the schedule is flexible, the work can be fast-paced and emotionally demanding. Some days are spent helping a client enjoy lunch or attending a routine doctor's appointment. Other days involve hospitalizations, family conflict, difficult decisions, or urgent care coordination. Flexibility goes both ways, and care managers must be comfortable adapting as client needs change.
