SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
This opportunity involves providing in-home and community-based behavioral health services to youth with mental health, behavioral, or substance use needs, as well as those involved in the juvenile justice system. Services are primarily delivered in Union County.
* Provides in-home and community-based outreach services to assigned youth.
* Assists in the creation and implementation of individualized behavioral interventions.
* Maintains a caseload and delivers consistent case management support.
* Completes timely, accurate documentation, including progress notes and care plans.
* Participates in the development and monitoring of Individualized Service Plans (ISPs).
* Facilitates skills-focused sessions and therapeutic activities for youth and families.
* Conducts outreach to re-engage families and youth who miss scheduled appointments.
* Collaborates with multidisciplinary team members and external service providers.
* Builds and maintains community relationships to enhance service linkage and referrals.
* Uses a person-centered, trauma-informed approach to help youth and families meet their goals.
* Attends ongoing training related to youth mental health, service planning, and behavioral health interventions. CLIENT'S REQUIRED EXPERIENCE & EDUCATION
* Bachelor's degree in Human Services, Social Work, Psychology, or a related behavioral health field required
* Minimum of 2 years of experience strongly preferred
* Valid driver's license
* Provide 2 professional references
