Physical Therapist / Physical Therapy Assistant (Home Visits – Full-Time or Part-Time) Location: Atlantic and CAPE MAY COUNTY / Flexible Schedule | Competitive Pay | Personalized Care Model

Our team is expanding and seeking a compassionate and self-motivated Physical Therapist (PT) or Physical Therapy Assistant (PTA) to deliver in-home therapy services. Role Responsibilities:

• Travel to patients’ homes to provide hands-on, evidence-based therapy

• Conduct assessments and develop customized care plans

• Collaborate with physicians, caregivers, and other providers as needed

• Educate patients and families on techniques to improve mobility and independence

• Ensure accurate documentation and compliance with HIPAA and care standards

We’re Seeking:

• Licensed PT or PTA with state credentials

• Experience in home health, geriatric care, or outpatient settings preferred

• Reliable transportation and ability to work independently

• Excellent communication and clinical judgment skills
