• Contract Duration: 13 weeks (through 06/13/2026)
• Schedule: Part-time; Tuesday & Wednesday required (an occasional third day may be offered based on program needs and RN availability)

• Shift Duration: 8-hour day shift
• Hours: 8:30 AM – 5:00 PM
• Hours per Week: 16
• Pay Rate: $88.35/hour
• Estimated Weekly Pay Range: $853 – $948

Job Requirements (Registered Nurse / Community Health RN)

• License: Active New Jersey RN license or NLC multistate RN license
• Experience: 1+ year RN experience with the elderly population ( community nursing experience preferred )
• Certification: AHA BLS (Healthcare Provider)
• Driver’s License: Valid driver’s license required (for occasional community-based responsibilities)
• Skills: Strong clinical assessment and triage, care planning and care coordination, patient & family education, and accurate EMR/EHR documentation

Responsibilities (PACE RN / Geriatric Nursing)

• Perform comprehensive nursing assessments and triage participant needs in the center and as needed in the community
• Partner with an interdisciplinary care team to develop, implement, and manage individualized care plans
• Provide participant and caregiver education on acute and chronic health conditions
• Communicate changes in condition and coordinate transitions of care to support continuity and safety
• Maintain accurate electronic documentation and clinical records in alignment with PACE program standards
• Support clinic/center-based workflows and provide occasional community-based nursing support (community-based visits) as needed
• Participate in quality improvement initiatives to enhance outcomes and the participant experience
