Job Overview:
The Community Case Manager will play a vital role in supporting individuals within the community by initiating and coordinating medical care and services for underserved and marginalized populations through increased health literacy and improved access. This role is essential for improving patient outcomes and ensuring individuals receive comprehensive and effective care. The CCM health professional will focus on disease prevention and delivering education and resources that allow a community population to improve general wellness through sustainable behavior change.
Qualification
Required:
- Prior experience in case management, healthcare and/or social services a plus.
- Strong organizational, communication and problem solving skills and the ability to work independently and as part of a team.
- Ability to work effectively with people from diverse cultures and diverse socioeconomic situations.
- Good working knowledge of local social service resources or skills to acquire and use this knowledge and information expeditiously.
- Desire to work with the HIV positive community and carry out the mission of the Center.
- NJ Driver’s license and access to reliable transportation or willingness to frequently utilize public transportation for community activities.
- Flexible/non-traditional work schedule is necessary.
Preferred:
Bachelor’s degree in Nursing, Social Work, Public Health or related field preferred.

Essential Functions:
Case Coordination
- Provide appropriate status neutral referrals to essential support services through linkage
- Provide assistance to clients with finding a medical provider and navigating through medication payment options including patient assistance programs, insurance enrollment and prior authorizations.
- Monitor the dedicated Prevention telephone line for all appropriate follow up and referral management.
Patient Advocacy
- Identify target populations and assesses sexual health education needs
- Promote and refer to syndemic testing for community member of unknown status
- Support rapid linkage, re-engagement and retention in HIV care and treatment for patients established as positive in the community
- Reduce/eliminate medical disparities associated with social determinants of health
Resource Management
- Manage supply inventory, prepare, deliver and monitor FTC Bathroom to Bedroom initiative prevention baskets
- Establish a functional knowledge of relevant local social service resources for referral to as appropriate
Documentation
- Evaluate and prepare reports as to the degree of success in achieving the predetermined objectives.
- Maintain accurate and complete participant records, tracking contact, participation, outreach, counseling, testing, and referrals performance
- Ensure the ongoing collection, collation and organization of data related to project resource and referral information
Education
- Recruit for and facilitate all 4 versions of Kiss & Tell educational workshops for designated priority populations, an average of 2 groups monthly required.
- Ensure Kiss & Tell group participants receive and complete follow up surveys at both 90 and 180 day intervals post workshops.
- Provide patients and clients with basic introductory education regarding PrEP/PEP.
- Improve health literacy levels to support patients in informed decision making processes pertaining to healthcare services
Collaboration
- Establish relationships with community organizations receptive to program services
- Promote and participate in related community and health events as needed
- Develop close working relationships with leadership and direct staff at community healthcare providers to ensure coordination of services
- Serve as a communication liaison between Ryan White and Non Ryan White funded medical providers and both clinical and non-clinical settings
- Implement and manage the FTC Bathroom to Bedroom initiative, an HIV and STI Self-Test Kit dissemination project
