What You'll Do
Care Coordination & Client Support
- Coordinate comprehensive case management services for Behavioral Health clients.
- Assist clients with housing stabilization, discharge planning, and access to community resources.
- Connect clients to recovery-oriented services, housing programs, financial resources, and supportive services.
- Develop individualized service plans, monitor progress, and adjust care plans as needed.
- Encourage and support client participation in recovery and wellness programs.
Collaboration & Advocacy
- Serve as a liaison between clients, treatment providers, community agencies, and other stakeholders.
- Coordinate referrals and ensure continuity of care throughout the recovery journey.
- Advocate for clients to help remove barriers to treatment, housing, and essential services.
- Participate in multidisciplinary treatment team meetings and aftercare planning.
Documentation & Compliance
- Maintain accurate and timely client documentation in HMIS/Credible and other required systems.
- Prepare and review client records to ensure compliance with licensing standards and organizational policies.
- Monitor documentation for accuracy, completeness, and quality.
Community Engagement
- Build strong relationships with community providers, housing organizations, and recovery partners.
- Stay informed about community resources and emerging behavioral health trends.
- Educate clients and community members on substance use disorders (SUD), co-occurring disorders, and available recovery resources.
Safety & Professional Development
- Help maintain a safe, clean, and welcoming environment for clients, staff, and visitors.
- Participate in emergency drills, staff meetings, and ongoing professional development.
- Complete additional duties that support the mission and success of the Behavioral Health program.
What We're Looking For
Required Qualifications
- High School Diploma or GED.
- Minimum of 2 years of case management experience .
- Strong organizational, communication, critical thinking, and problem-solving skills.
- Working knowledge of behavioral health, addiction, and recovery principles.
- Proficiency with Microsoft Office (Word and Excel).
- Valid driver's license.
Preferred Qualifications
- Bachelor's or Master's degree in Social Work or a related human services field.
- CPRS, CADC, CSW, or other relevant behavioral health certifications.
