- Provide in-home physical therapy evaluations and treatments to patients with orthopedic, neurological, pediatric, and geriatric conditions
- Develop and implement individualized treatment plans to help patients achieve their functional goals
- Collaborate with Occupational and Speech Therapists, as well as other healthcare professionals, to ensure coordinated care
- Educate patients and caregivers on exercises, safety, and ongoing rehabilitation techniques
- Accurately document patient progress and maintain clinical records in compliance with company and regulatory standards
- Communicate effectively with patients, families, and the care team to ensure positive outcomes and satisfaction
